If you are a performer who lives and works in the United States, we have to keep a W-9 form on file so we can send you earned income statements at the end of the tax year. If you are from outside the US, you can usually skip this step.
First, visit the verification page.
On this page, you can download a blank form W-9. You will need to fill and sign this document. Adobe Fill & Sign app can work well for this and is available for mobile as well as PC, or an iPhone app W-9 Form - PDF Template will also work. Some models prefer to print out the W9 form, fill it out and sign it and then scan it in to upload.
You can then upload this document to us on the same “verification docs” page.
